Fees & Payment Info
Consultation: I can provide a initial phone consult free of cost to determine if I may be a fit for your child or family.
Rate: For a complete review of fees and payment policies please contact me at (510) 847.8292 or email@example.com. Fees are due on either weekly or monthly basis. Payment is accepted via credit card, personal check or cash.
Reduced Rate: I have a sliding scale available for families that cannot afford my full fee. Please inquire if you would like to learn more.
Cancellation: Regular attendance at your sessions is vital to our work together. I have a 24 hour cancellation policy and once our standing appointment is scheduled, youʼll be required to pay for the full cost of your session unless you provide at minimum 24-hr advance notice of cancellation.
Insurance: Services may be covered in full or in part by your health insurance or employee benefit plan. You are responsible for making the necessary arrangements with your insurance provider and initiating the process. Please check your coverage carefully by asking the following questions:
Do I have mental health insurance benefits?
What is my deductible and has it been met?
How many sessions per year does my health insurance cover?
What is the coverage amount per therapy session?
Is approval required from my primary care physician?
